New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photo ID which you need to present in person.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Links to all relevant forms are below and can be printed off and brought along to the surgery.
When filling out the registration form it is important you fill in all information required if you do not have your NHS number you can obtain this from your previous surgery, address details must include postcodes.
GMS1 Registration Form
New Patient Questionnaire - This form must be completed in full for registration to go ahead, please pay attention to sections about parental responsibility and consent especially if you are completing the form on behalf of another person.
If you are on any regular medication you should initially make a double appointment with a GP to go through your medications.
