Clinical Practitioners

Nurse Practitioners are able to take a medical history, carry out medical examination, make a diagnosis, organise investigations and offer treatment and advice for a variety of common health conditions. Examples of these are sore throats, ear infections, cough, skin conditions, stomach upsets, chest infections and contraceptive queries.

LINDA GREEN

Lead Nurse

Is an experienced clinician who has undergone extensive additional training and works closely with the surgery GP’s. They act within their professional boundaries, providing urgent care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care/referral within the general practice. They work collaboratively with the GP’s and general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

SAM PERKINS

Is an experienced clinician who has undergone extensive additional training and works closely with the surgery GP’s. They act within their professional boundaries, providing urgent care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care/referral within the general practice. They work collaboratively with the GP’s and general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

CLAIRE DUGGAN-SWIFT

Paramedic

Is an experienced clinician who works closely with the surgery GP’s. They act within their professional boundaries, providing Home visits for patient’s unable to get to the surgery for urgent care assessment and management. They work collaboratively with the GP’s and general practice team to meet the needs of those patients.

JO PHILIPS

Is an experienced clinician within mental health who, acting within their professional boundaries, will provide acute mental health care for the presenting patient from initial history taking, clinical/mental assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for mental health patients within the general practice.

Nurses

MIRANDA BURKE

Is a registered nurse who is responsible for ensuring the delivery of safe and effective nursing care. They manage patients with a long term condition, carry out immunisations, smears, wound assessments, complex dressings as well as general nursing duties. They work under the direction of the nurse lead, Practice Manager and GPs to deliver the practice priorities.

REBECCA SEABROOK

Is a registered nurse who is responsible for ensuring the delivery of safe and effective nursing care. Senior nursing assistant working under supervision from the nurse lead carrying out specific tasks, clinical activities and tests for named patients, within the scope of established practice protocols, policies and procedures, including venepuncture, urine testing, blood pressure readings, ECG's, assisting with minor ops, ear irrigation, INR checks, simple wounds, removal of sutures/clips and health checks.

Phlebotomists

Is a trained member of the team who works under the direction of Nurse Lead and takes blood from patient for investigations . They also carry out warfarin blood checks, baby checks and BP monitoring.

Practice Manager

The practice manager is involved in managing all of the business aspects of the practice such as making sure that the right systems are in place to provide a high quality of patient care, human resources, finance, patient safety, premises and equipment and information technology. The practice manager supports the GPs and other medical professionals with delivering patient services and also helps to develop extended services to enhance patient care.

Administers the practice accounts and deals with variable financial matters.

Reception Manager

Supervises the reception team and undertakes other admin duties. Karen is also the first point of contact within the surgery if you have a complaint.

Administrator, responsible for all administration regarding childhood immunisations, adult immunisations, coils, breast screening, deaths and more.

SUE TAYLOR

Nurses’ Clerk, dealing with all aspects of administration for the nurses and healthcare assistant. Looks after the day to day running of IT for the practice.

Prescriptions Clerk Lead - works closely with the pharmacies.

Responsible for dealing with the READ-coding of information regarding child protection issues. Also assists the practice summariser.

Community Midwives

The community midwife provides care during and immediately after pregnancy at the surgery or home. Monday is for booking appointments (1st ante-natal appointment) Thursday is for routine ante-natal appointments. In times of absence or sickness within the midwife team and you need to be dealt with urgently you can contact Four Woods on tel no. 01752 366795 or you can call the maternity message handling service on 01752 517888. One of the midwives will be able to answer any questions regarding appointments and fit any ladies in who are unable to get appointments at the surgery.

Community Nurses

The district nurses team are available to help housebound patients when necessary. Requests for visits should normally be made through the surgery. If it is necessary to contact them direct, ring 01752 436716 during normal working hours or if it is out of hours call 0845 2419 130 or 01392 269475.

Counsellors

Improving Access to Psychological Therapies (IAPT). Available to see patients at the surgery for counselling. Referrals are made through the surgery to the IAPT Team. You can also self-refer by phoning 01752 -314520.

Health Visitors

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing. St Lukes

Members of the Plymouth Community Specialist Palliative Care Team work closely with our surgery. They can be contacted on 01752 314360 or through the surgery.
