The Outpatient Physiotherapy service has agreed with Local Commissioners to offer a self-referral option to access their service. After a successful pilot with three local GP practices the next stage is to expand this option slowly across the whole Plymouth Health Community.  We can now offer this service at our Practice.

To self-refer either:

• Pick up self-referral form from your GP surgery. Fill in form fully and send to Physiotherapy department, Zone E, Level 7, Derriford Hospital Pl6 8DH.
• Fill in form on line at Plymouth Hospitals Physiotherapy Self Referral
• Or Type in “Plymouth Hospitals Physiotherapy self-referral” into a search engine to find the form.
